The Ardens Psoriasis Template allows the clinician to record information of the patient's general history, examination findings and management advice.
Access the template within the patient record in one of the following ways:
- Click on the Run Template option within the consultation.
- Click on the Add > Data using Template option outside of a consultation.
Clicking on either of the options above will display the Template Picker screen.
Search Psoriasis using the search box provided or via the navigation pane on the right. Select the template and click OK.
The template has been broken down by pages on the left-hand side. Navigate to the page required.
The Conditions page provides general information about psoriasis.
The History page enables the clinician to record relevant history information including the impact these symptoms have on the patient's quality of life and wellbeing. There is also a section to record whether there is an increased risk of cardio-metabolic disease.
The Lifestyle page allows you to capture information relating to smoking, alcohol, drugs, and occupation. Please note - any information displayed in red writing relates to payment purposes such as QOF or Network DES.
The right hand side of the data fields, will display information of the latest coded entry if recorded for the patient. Clicking on the arrow to the right of this will open a more detailed view.
The Examination page allows recording the diagnosis, observations, and information about the physical examination. This page also includes a clinical diagram enabling the user to document and annotate a body map. For more information on how use the body map, please click here.
The Management page enables you to record advice given to your patient as well as recording possible complications.
The Review page should be used for patients who have an established diagnosis of psoriasis, enabling you to document a plan, follow up information and recording any changes in treatment. You may also wish to refer to other pages in this template for more data collection, in particular the History, Examination and Management pages.
The Resources page includes many relevant links to resources for patients, as well as links to websites for healthcare professionals.
Complete the template using the required fields and ensure you save the information, using the Save option.
If using the template via the Run Template option within an open consultation, you will also need to click Save to save the consultation.
