The Ardens Frailty and the over 75s Review Template allows the user to record comprehensive details of overall health in an over 75 patient, including mobility, falls, frailty, dementia, as well as recording care plan details and medication reviews.


Access the template in one of the following ways:

  • Click on the Run Template option within the consultation.
  • Click on the Add > Data using Template option outside of a consultation.


Clicking on either of the options above will display the Template Picker screen. 

Search for the 'Frailty and the over 75s Review' template using the search box provided or via the navigation pane. Select the template and click on OK.


The Frailty and the over 75s Review template is designed to simplify data collection for elderly patients living in their own homes. Ardens has created a separate Care Home Review template for care home patients.  


You may also wish to use the following templates:  Future Care Plan – Palliative Care, Universal Care Plan, Anticipatory Prescribing, Mental Health, Falls Assessment, as well as individual Chronic Disease templates.


The Frailty and the over 75s Review template has been broken down by pages on the left-hand side. Navigate to the page required.


Use the Administration page of the template to record information such as type of review, residential status and consent to share information with third parties.

The Baseline/Initial Assessment page is used to record the current status of the patient in terms of daily living abilities, mobility, vision and hearing, skin and bone health, mental state, nutritional wellbeing, frailty and overall condition and pain status.  The page following this can be used for Depression / Anxiety Screening.

The Dementia Assessment page may be used to record initial screening for dementia, review of dementia care plan and referral to memory clinic. There are also links to assessment tools such as 6CIT, Mini-Cog and Test Your Memory.

Please note that to record a new diagnosis of dementia, or to record a dementia annual review, the Ardens Dementia Template should be used.


The Rockwood Frailty Scale page facilitates the recording of a frailty scale result. There are suggestions for clinical and risk assessments, care planning and onward referrals, depending on the patient’s frailty score. In addition, there is the ability to record a clinical diagnosis of frailty.

The Acute Visit page can be used to record information from a weekly assessment or an acute visit, such as history, observations, examination and management plan details.

The Care Planning page allows the user to record the creation or review of a Personalised Care and Support Plan in support of the Network Contract DES EHCH-02 requirements.  This includes general and future care planning, next of kin, legal information, treatment escalation planning, patient considerations, CPR decisions and date of next care plan review.


The Medication Review page enables the recording of a structured medication review (SMR). If a more comprehensive SMR is required, please use the Ardens Structured Medical Review template.


The Referrals page facilitates the recording of multiple referrals, if required, including a referral for social prescribing.

The Vaccinations page can be used to refer to the patient’s record of influenza, pneumococcal, shingles and COVID 19 vaccinations, as well as recording allergies, contraindications and declines.

The MDT Approach page can be used to record multi-disciplinary team discussions and referrals made.

Complete the template using the required fields and ensure you save the information, using the Save option.


If using the template via the Run Template option within an open consultation, you will also need to click Save to save the consultation.


For practices that are on either the Ardens Plus or Pro packages, local reporting requirements maybe included in the templates. This local content will be visible in standard Ardens templates, as pages or sections when viewed via a patient record.


Below is an example of local content as a page in the standard Frailty and the over 75s Review template:

Ardens has created an optional protocol (Add eFI score to record) which can be used to automatically add a frailty, or electronic frailty index (eFI) score to patients over the age of 65.  This is a calculation based on data within the patient record. The score should be used as a guide only; clinical judgement is needed before a diagnosis is made.  The protocol can be useful to help identify patients that should be reviewed for frailty.
