The Ardens Wound Management and Leg Ulcers Template allows the user to record comprehensive details of wounds and leg and pressure ulcers, including wound infection, Tetanus status, referrals and management plans.

 

Access the template in one of the following ways:

  • Click on the Run Template option within the consultation.
  • Click on the Add > Data using Template option outside of a consultation.

 

Clicking on either of the options above will display the Template Picker screen. 



Search for the 'Wound Management' template using the search box provided or via the navigation pane. Select the template and click on OK.

 

The template has been broken down by pages on the left-hand side. Navigate to the page required.

 

The History, page allows the user to record broad details of the wound, its date of onset and any pain present, together with prescription entitlement details.



The Lifestyle and Examination – general pages enable the updating of the patient’s smoking and alcohol statuses, BMI, blood pressure and heart rate, as well as there being a link to a Malnutrition Universal Screening Tool (MUST) to calculate a score for a patient’s nutritional status.



The Examination – wound page can be used to record specific details about a wound.  There are also links to wound assessment tools such as TIMES Wound Assessment and Waterlow Wound Assessment.



The Examination – body map page allows the user to document and annotate body maps as part of your patient's care. There are many body maps available. It can be useful to store an annotated diagram in a patient record, either for comparison purposes, or for inclusion in a referral letter.

Where there may be a Tetanus risk, the Tetanus status page gives the user information to assist in identifying the tetanus status of the wound. There is also a link to the Green Book chapter for Tetanus.



If infection is present, the Wound infection page can be used to record any signs of infection, swabs and follow up actions taken.



If wound care has been initiated by a secondary care provider, the Post-operative wound care page can be used to record these details.

 

The Leg ulcers and pressure ulcers page allows the user to record specific details about ulcers, including ABP indices and Waveform signals.



The Management and follow up page allows a plan to be recorded for wound or ulcer management, lifestyle changes, dressings, suture removal and a diary entry for follow up.



Complete the template using the required fields and ensure you save the information, using the Save option.

 

If using the template via the Run Template option within an open consultation, you will also need to click Save to save the consultation.

 

For practices on either the Ardens Plus or Pro packages, local reporting requirements maybe included in the templates.  This local content will be visible in standard Ardens templates, as pages or sections when viewed via a patient record.
