Due to the complexity of the CVD-01 requirements, Ardens are unable to provide an accurate search to monitor achievement for this indicator. The one we have provided will be an indication as to achievement but will not match the GPES extract exactly.

To support practices to identify the work to be done towards this indicator, a suite of "work to do" searches have been produced.


To locate the searches navigate to the Population Reporting module > Ardens Searches > 5.3 Ardens National Contract Reports > Network Contract DES (NCD) (2022-23) > a. Investment and Impact Fund (IIF) > 2. Work to do > Cardiovascular disease > CVD-01 Work to do.





1a. High BP in 2 years before 01.04.22, need BP since 01.04.22

This search identifies all patients with a high BP recorded 2 years before the 1st April 22 and the patient has NOT had a BP recorded since 01.04.22. 


ACTION - arrange for the patient to have a repeat BP.  Run and view the results of the associated report for information on the latest BP recorded for the patient. 


Please note - these patients need a BP at some point in the fiscal year -any patient that still has a high BP reading once repeated will fall into search 1b for further follow up.


Warning - this search may produce high results, we suggest exporting the report as a CSV file and further filtering the order of patients, e.g. sort by date.  


1b. High BP in 2 years before AND since 01.04.22, need follow-up

This search identifies all patients with a high BP recoded 2 years before the 1st April 22, and the patient has had a further high BP recorded since 1st April.


ACTION - patient needs to be followed up. Run and view the results of the associated report for information on the latest BP recorded for the patient.

2. Low or No BP in 2 years before 01.04.22, high BP since, need 6m follow-up

This search identifies all patients with a low BP or no BP recorded 2 years before the 1st April 22, however, since 01.04.22 the patient has had a high BP.


ACTION - patient must be followed up within 6 months from the earliest recording of the high BP (where there is not a low BP on the same date).  Run and view the results of the associated report for information on the BP recording for the patient and any follow up which has been done. As it is not possible to accurately exclude patients who have had follow up there may be some here who do not need further work - the report will make this clear as they will have one of the following showing in the report:

  1. Antihypertensive medication prescribed and added to QOF hypertension register after 01.04.22
  2. Antihypertensive medication declined and added to QOF hypertension register after 01.04.22
  3. Home BP or ambulatory BP readying after 01.04.22
  4. Referral for hypertensive emergency recorded on the same date as the earliest high BP since 01.04.22 where there is not a love BP on the same date.

 

Please note - if you come across patients that have had a high BP recorded but a subsequent low BP recorded on the same day, you do not need to follow-up these patients.
