The Domestic Violence + Abuse template facilitates the detailed recording of information in relation to a patient’s disclosure of violence or abuse, as either a victim or perpetrator.
This template relates only to adults and young people aged 16 and over. Please also refer to the Ardens Safeguarding templates for Adults and Children.
Access the template in one of the following ways:
- Click on the Run Template option within the consultation.
- Click on the Add > Data using Template option outside of a consultation.
Clicking on either of the options above will display the Template Picker screen.
Search for the "Domestic Violence + Abuse" template using the search box provided or via the navigation pane. Select the template and click on OK.
The Background page outlines what types of behaviour are covered by the term "domestic violence or abuse" and gives statistics on the extent of domestic abuse in England and Wales in recent years.
The Confidentiality/record-keeping page is used to record that the patient has been informed of the confidentiality of the information they provide, and that such confidentiality may need to be broken in exceptional circumstances. There is also a section relating to recording-keeping. This includes links to a RCGP article about recording domestic abuse as well as a note on the use of the ‘Online Visibility’ option within an open consultation in the clinical system.
The History page allows the clinician to use screening and risk assessment tools such as WAST and DASH. Information relating to a patient’s living arrangements, presence of children in the household and risk factors can also be noted.
The Type of domestic abuse page facilitates the recording of the nature of the abuse, whether that be physical, emotional or psychological, verbal, sexual, financial, honour-based or technological.
The Social history page allows the patient record to be updated for smoking status, occupation, and alcohol or illicit drugs use.
The Examination page is used to record verbal consent for examination or medical photography. There are also a number of different body maps which can be annotated as required.
The Disclosure by perpetrator page has links to articles which can help healthcare professionals who are working with perpetrators to record details of abuse. There are free text boxes to record history and plan details. The Respect phoneline number is also quoted here.
The Immediate danger page provides advice to the healthcare professional about responding to disclosure of domestic abuse. There is also information on the ‘silent solution’ for when a person has called 999 for help but cannot speak due to danger.
The Management page can be used to record advice given, planning discussions, and any referrals made.
The Resources page offers links to information resources for everyone, and is broken down into resources for women, men, children and young adults, perpetrators, and healthcare professionals.
Complete the template using the required fields and ensure you save the information, using the Save option.
If using the template via the Run Template option within an open consultation, you will also need to click Save to save the consultation.
