The Ardens Sepsis or Sick Person template serves as a framework for a thorough assessment of a potentially sick person.  It has been produced with the co-operation of The UK Sepsis Trust, and in line with the Sepsis Toolkit.


The template is for use with patients aged 5 or over, as well as for pregnant patients and patients up to 6 weeks post partum.


For patients aged 0-4 years of age, please use the Ardens ‘Fever in child < 5 years’ template.


Ardens has also provided a ‘Sepsis screening – telephone triage’ template.


Access the Suspected Sepsis or Sick Person template in one of the following ways:

  • Click on the Run Template option within the consultation.
  • Click on the Add > Data using Template option outside of a consultation.


Clicking on either of the options above will display the Template Picker screen. 

Search for the "Suspected Sepsis or Sick person" template using the search box provided or via the navigation pane. Select the template and click on OK.

The Condition page gives an overview of the condition and instructions for using the template. 


Thereafter, the template pages are divided into 3 categories:

  • Children aged 5 – 11 years of age
  • Children 12+ years and adults
  • Pregnant patients, and patients up to 6 weeks post partum


For each category, there are 3 pages to complete:

  • History
  • Exam
  • Management


Use the template pages that are relevant to the category of patient you are assessing for infection.


The images below show the pages for the Children 12+ years and adults category.


Use the History: 12+ YEARS/ADULT page to record relevant history.  The first section includes some points for consideration and questions to ask.   You may also record any risk factors for sepsis relevant to this age group, as well as recording whether the patient is immunocompromised or not.  

The Exam: 12+ YEARS/ADULT page facilitates the adding of current observations, such as temperature, blood pressure, heart and respiration rates, and oxygen levels. You may also record other examination findings, for example the patient’s appearance and colour.

The Management: 12+ YEARS/ADULT page lists red and amber flags for sepsis in this category of patient and gives advice on the action to take depending on the flags present.

There is also a section to record the plan of action to be taken, any follow-up to be arranged and advice given.

The Follow up of sepsis survivors page includes links to patient information leaflets and complications of sepsis.

The Resources page has a link to the NHS website for Sepsis symptoms, as well as links to relevant websites for health care professionals.

Complete the template using the required fields and ensure you save the information, using the Save option.


If using the template via the Run Template option within an open consultation, you will also need to click Save to save the consultation.
