The "Ardens Asylum seekers and marginalised groups template" is used to record a detailed assessment of the health and wellbeing needs of asylum seekers or other marginalised groups, such as refugees or the homeless. It has been updated to include the most recent guidance to support refugees from Ukraine.


Access the template in one of the following ways:

  • Click on the Run Template option within the consultation.
  • Click on the Add > Data using Template option outside of a consultation.


Clicking on either of the options above will display the Template Picker screen.

Search for the "Asylum seekers and marginalised groups" template using the search box provided or via the navigation pane. 

Select the template and click on OK.

The Background page provides definitions for the groups of people to which this template relates to. It also confirms entitlement of these groups to receive primary care free of charge, once registered with a GP practice in the UK.


The Administration, Medical information and Medications pages are used to record information such as ethnicity, place of origin, language, medical background, medications and allergies.  The image below is of the Administration page.

There are then pages to record Social History, Lifestyle, Alcohol screening, GPPAQ and QRisk2. The image below shows the Social History page.

The Examination page allows for the recording of examination done, blood pressure taken, other general examinations, and basic measurements to be added to the patient record.

The Screening page allows for the recording of advice given in relation to screening programmes such as cervical, bowel and breast cancer, as well as the AAA screening programme and testicular exam advice for male patients.

The Infectious disease page provides guidance for screening and assessment of patients arriving from countries abroad, including specific guidance for arrivals from Ukraine.

The Contraception + sexual health page provides the opportunity to record contraception status, pregnancy assessment and sexual health history.


The Immunisations page is used to record whether the patient is up to date with regard to the UK vaccination schedule, and with COVID vaccinations.


The Safeguarding page can be used to record history and risk in relation to various safeguarding issues. Concerns and vulnerability can be recorded, as well as any referrals or planned actions.

The Resources page has links to various resources, for both patients and for healthcare professionals.

Complete the template using the required fields and ensure you save the information, using the Save option.


If using the template via the Run Template option within an open consultation, you will also need to click Save to save the consultation.
