Cholesterol forms part of the clinical domains area of QOF.

 

There are 2 Cholesterol QOF indicators:

 

CHOL001 - includes patients on the QOF CHD, PAD, Stroke/TIA or CKD register, who in the last 6 months of the fiscal year are prescribed a statin, or if a statin is declined or clinically unsuitable another lipid-lowering therapy.  Patients aged 17 years or over, on any of these registers who also have a diabetes diagnosis are excluded from this indicator.

 

Personalised care adjustments (PCA) apply if they meet and have been appropriately coded with any of the following:

 

  • have a palliative care code on their record on or after 1/4/2008.
  • they decline or are clinically unsuitable for both statin and all alternative lipid-lowering therapy this fiscal year.
  • they have been coded as on maximal tolerated cholesterol lowering treatment this fiscal year.
  • they have been coded with a lipid lowering therapy adverse reaction, or a lipid lowering therapy not indicated/contraindication/decline code this fiscal year.
  • they have been newly registered at the practice in the last 3 months of the fiscal year.

 

 

CHOL002 - includes patients on the QOF CHD, PAD or Stroke/TIA register who have a recording of non-HDL cholesterol this fiscal year that is lower than 2.5mmol/L, or where non-HDL cholesterol is not recorded a recording of LDL cholesterol this fiscal year that is lower than 1.8mmol/L.

 

PCA applies if the patient declines cholesterol blood test this fiscal year, or the patient has registered within the last 9 months of the fiscal year.

Clinical Templates

The following templates are available to support staff with recording QOF activity for cholesterol.

 

  • Chronic Disease Templates - the Ardens CHD, PAD, CKD and Stroke/TIA templates all contain a QOF only page for capturing all the QOF requirements for the current financial year.
  • Multi-Morbidity Templates – for patients with multiple long-term conditions and to capture all QOF requirements for all conditions relevant to the specific patient.

 

Searches

The following search folders are available to support staff with achieving and monitoring your QOF cholesterol indicators.

 

  • QOF Monitor Dashboard – works with the Ardens Manager QOF dashboard to allow a visual of QOF performance in monetary and points values.
  • QOF Misc Searches – contains a suite of Data Quality and Work to do searches to help with achievement of these indicators.
