The Ardens Carer Review template helps to record brief details of a carer’s health and wellbeing. You can record their carer status, health checks and any referrals made.
Access the template in one of the following ways:
- Click on the Run Template option within the consultation.
- Click on the Add > Data using Template option outside of a consultation.
Clicking on either of the options above will display the Template Picker screen.
Search for the 'Carer Review’ template using the search box provided or via the navigation pane. Select the template and click on OK.
The Carer Information page allows the recording of carer status, ethnicity, offer of annual health check and flu vaccination, as well as any support that the carer may be signposted towards.
The Carers Health Check page advises encouragement to take up the NHS Health Check (if the carer is eligible). If the carer is not eligible for a NHS Health Check, you may use this page to record health details such as BP, smoking status and cessation advice, alcohol consumption and BMI.
The Referrals/plan page facilitates the recording of any referrals and plans made, such as to Social Services for a Carer Assessment.
Complete the template using the required fields and ensure you save the information, using the Save option.
If using the template via the Run Template option within an open consultation, you will also need to click Save to save the consultation.
Carer Status QOF Alert
This alert will display information on the carer and cared for status for patient within the QOF alert box. Clicking on the alert will launch a template should the carer status need to be updated or changed.
