Ardens has created a wide range of clinical templates to assist with data entry for a variety of clinical areas.

 

The available Obs and Gynae Templates include: 

  • Amenorrhoea – enables you to record both primary and secondary amenorrhoea including guidance on when to refer the patient and links for both patients and care provider advice for polycystic ovaries and menopause.
  • Ante-natal – use this template to record the ante-natal care at each trimester and outcome of the pregnancy. Influenza and pertussis vaccinations can also be recorded too.
  • Female Genital Mutilation – FGM is considered a grave violation of the rights of girls and women and can cause serious consequences for a women’s health. This template can be used to record type of mutilation and has guidance when to refer and to whom.
  • Fertility Problems – history and lifestyle may affect fertility and can be recorded in this template, along with advice, referrals and useful hyperlinks for the patient.
  • Menopause and HRT – general menopause advice and the initiation or review of HRT can be recorded in this template, with useful links to NICE guidance, FRAX and QFracture information.
  • Menorrhagia and Dysmenorrhoea – enables you to record the history, examination and management for menorrhagia and dysmenorrhoea. . The template includes links to a period diary, general advice for the patient and useful links for the health care professional
  • Nausea/Vomiting in Pregnancy/Hyperemesis Gravidarum – includes links to patient information leaflets and websites with advice around managing nausea.
  • Pain/bleeding in early pregnancy – enables you to record the history and examination of a patient in early pregnancy.. Additionally, there is a page to record advice, with links for both patient and the care provider if there is a miscarriage.
  • Pelvic Inflammatory Disease – includes information to record safety netting, contract tracing and screening.
  • Postmenopausal bleeding – this template can be used as a a good aide-memoire for a patient presenting with postmenopausal bleeding, including guidance on when to consider a fast track referral.
  • Postnatal Mother –enables recording of safeguarding, mental health and contraception advice, recording any problems with baby feeding, general lifestyle and sleep. There is a 6-8 week baby check template, which can be found in the Ardens Paediatric template folder.
  • Pre-conception – enables you to record any advice given re pre-conception and record history and examination. Links to planning a pregnancy guidance and the genetic risk tool are included in this template too.
  • Pregnancy likelihood – a simple template to record an assessment of how likely a pregnancy may be.
  • Prolapse and Pessary – enables the recording of an insertion or a review/renewal of a pessary, it includes useful RCOG guidance and patient information leaflets and website links
  • Vulvovaginal Candidiasis – this condition is often over diagnosed , this template gives current guidance on diagnosis including history examination and links to useful leaflets and websites.


The Obs and Gynae templates are located in the Ardens Live Toolset > Clinical Templates folder.

 

They may be accessed within the patient record in one of the following ways:

  • Click on the Run Template option within the consultation.
  • Click on the Add > Data using Template option outside of a consultation.

 

Clicking on either of the options above will display the Template Picker screen. 



Search for the template you require using the search box provided or via the navigation pane. Select the template and click on OK.

 

Alternatively, you can launch the template using the Template Launcher (All Template Launcher or zTemplate launcher: Obs and Gynae protocols)

 

Once you have launched the template, complete the template using the required fields and ensure you save the information, using the Save option.

 

If using the template via the Run Template option within an open consultation, you will also need to click Save to save the consultation.
