Ardens has developed a number of resources to support practices achieving the Network Contract DES requirements for 2021/22. For further information on this, please click here. The following support article outlines different searches available to support the latest requirements.

To access the searches, navigate to the Population Reporting module > Ardens Searches > 5.3 Ardens National Cntrct Reports > Network Contract DES (NCD) folder.
Disclaimer: The Ardens National Contract Reports (including QOF, NCD, SMI, NHS HC) are to be used purely as an indicative guide for practices for practical purposes. They are not intended to replace the national reports. Whilst they are based on the business rules, they do not accurately reflect every rule.
A number of NCD indicators also use data from other sources that are not available via the standard EMIS Web search / report screens. Specific examples include:
- NCD012 - Data from manual submissions by PCNs via CQRS
- NCD013 Data from GP Appointment Data
This folder is broken down by sub folders for each requirement of the Network Contract DES for the current financial year.
Investment and Impact Fund (IIF)
The Investment and Impact Fund (IIF) folder supports practices with the IIF requirements. A summary of the IIF requirements can be found here.
Each sub folder will include a defined register of patients, activity achieved and will identify work still to be done.
The Care Home folder includes a search to display the number of patients recorded in a care home and the activity completed for the care home patients. The TO DO searches indicates were a personalised care & support plan and a structured medication review are not complete, once the searches have been run, click on the Population Included tab to display the patients that have outstanding work to do.
Practices may wish to use the Ardens Care Home Review template to record this activity.
The CVD folder includes a search to identify patients not on the Hypertension register that meet the criteria for a follow up (Population Included patients). This is further broken down into the 'Needs Follow-up' report, which once exported, will allow you to priorities patients by BP.
Searches are available (CVD-02) for practices to see if there has been an increase in the practice Hypertension register since 1/04/21.
Practices may wish to use the Ardens ECG + Screening template to record the outcome of the follow-up.
The Ethnicity, Access, and Social Prescribing folder will include searches to identify online consultations carried out (note that this may not be accurate number due to how the data is extracted), number of patients recorded with an ethnicity code and number of social prescribing referrals recorded since 1/04/21.
The TO DO search will allow you to carry out work on those patients that require an ethnicity code to be recorded. For further information on how to action this search using the Ardens Ethnicity resources, please see the following support article.
The Inhalers folder identifies the number of patients on low carbon inhalers. The Inhalers - Work to do folder will identify work to be done.
We suggest practices focus initially on search a) for each area, this will identify those patients heavily using inhalers. Once the searches have been run, the Population Included tab will display the list of patients.
Please note - there is no available code on EMIS for 'Dry powder inhaler not indicated', patients will therefore remain on the list until a code is in place.
The LD folder will display the number of Learning Disability patients with a health check and action plan complete, along with TO DO searches to identify outstanding work.
Practices may wish to use the Ardens Learning Disability resources to record the health check information and produce a action plan for the patient.
The Seasonal Influenza folder will display the number of patients by eligible cohort that have received the flu jab and information recorded correctly. Practices should be aware of those patients appearing in the 'DATA QUALITY - Wrong vaccination code searches' and update the patients with the correct code accordingly to receive payment.
Practice may with to use the Ardens Flu resources to correctly code patients that have received the flu vaccine.
Network Contract DES Indicators
This folder is broken down by two sub folders:
- Management Information - The Network DES business rules include a number of "Management Information" indicators (prefixed with NCDMI). These are not linked to payment but we understand that this activity data may be auto-extracted via GPES. We have included searches based on these rules so that practices are aware of what data may be extracted.
- Payment Indicators (IIF) - We suspect that most IIF activity will be auto-extracted using the "Payment Indicators" business rules (prefixed with NCD). You will notice that there is one missing - this is for the inhalers indicator. The data will be extracted using BSA data and we are unable to build anything approximating this within EMIS.
Severe Mental Illness
The Severe Mental Illness folder provides an audit of the number of SMI health checks done this fiscal year, along with a search to identify those patients needing a SMI health check.

Structured Medication Review
The Structured Medication Review folder supports practices to identify and prioritise patients who would benefit from a structured medication review.
The Target populations folder breaks the target population by groups (care home residents, patients recorded as having a fall or admission in last 6 months, housebound, medication associated with errors, severe frailty patients and those patients on 10 repeats or more.
The Work to do folder identifies work to be done and patients requiring a structured medication review. Click on the Population Included tab to identify the patients. Please note - the Ardens Structured Medication Review includes standardise coding of this activity.
The Cancer folder supports practices on the participation of national cancer screening programme and improvement on the referral and safety-netting practices for patients suspected of having cancer.
The Clinical Activity folder (once run) will report on diagnosis, referral and screening activity recorded for this fiscal year.
The Register folder will provide practices with detailed cancer registers.
The Work to do folder identifies work to be done. This is mainly patients that have not had screening advice recorded in this fiscal year. The practice also have the option to narrow down to specific groups of patients, such as Learning Disability, Transgender and BAME.
To complete the work to do, access the patients displaying in the Population Included tab, check if a patient has been given advice and code appropriately. The Ardens 'Cancer Screening Follow-up Template' includes relevant coding.
Please note - the 'work to do' search for fast track referrals will exclude those patients coded as declined or inappropriate referral.

The Care Homes folder supports practices to audit the activity for the Enhanced Health in Care Homes requirement.
This folder will provide practices with detailed practice care registers, including patients currently living in a nursing/care/residential home and those patients who have passed away in a care home in the past year.
The Care home data quality search folder identifies patients who have a 'care home' code in their record which is not recognised by the PCN DES business rules. You may wish to re-code these with 'Lives in residential home' or 'Lives in nursing home' ((or living temporarily in care home') as these are the recognised codes.
The Network Contract Requirements - work to do folder identifies outstanding work for Care Home patients.
zArchive - PCN DES 2020-21
This search folder includes data for last years Network Contract DES activity and is to be used for audit purposes only.
