The Ardens Long COVID Template offers guidance and support for practices managing patients with the long term effects of COVID-19. The template follows the latest NICE guidance for patients who have new or ongoing symptoms 4 weeks or more after the start of the acute COVID-19.


Access the template in one of the following ways:

  • Click on the Run Template option within the consultation.
  • Click on the Add > Data using Template option outside of a consultation.


Clicking on either of the options above will display the Template Picker screen. Search for the 'Long COVID' template using the search box provided or folder navigation pane. Select the template and click on OK.


The template has been broken down by pages on the left-hand side, navigate to the page required.


The Condition page lists the most common new or ongoing symptoms after acute COVID-19. 


To support healthcare professional assessing patients, the History page allows the clinician to complete the comprehensive clinical history. It includes details of the previous COVID-19 diagnosis and COVID-19 vaccination history if applicable for the patient.


Please note - at present there is no clinical code available to record Long COVID diagnosis. In the meantime, Ardens have used the code 'C/O-postviral syndrome' to be used for searching purposes. Once an appropriate code has been made available, the resources will be updated to reflect the change. 



The Depression/anxiety screening page links clinicians to the PHQ-2 Depression screening and GAD-2 Anxiety questionnaire tools.


The Examination page allows clinicians to complete observation and general examination details for the patient. 


The Investigations page outlines the recommended NICE blood tests and investigations to be offered tailored to the patients signs and symptoms.


The Management page provides clinicians with information around the self management plan. Please refer to the Resources page for supporting resources for patients requiring mental health, social, financial and cultural support.

 The Referrals page displays guidance on urgent referrals and general referral advice following NICE guidance. 


Complete the template using the required fields and ensure you save information, using the Save Template option.
