Ardens has designed care planning tools to support practices in managing palliative care patients. The template has been produced in association with the Future Care Planning Project. Completing the appropriate parts of the template will be updated to the patients electronic record.
In addition to the clinical template, an optional Future Care Plan document is available, which extracts all the relevant information, allowing a care plan to be left in the patient's house or shared (with consent) with other agencies and carers as appropriate.
Access the Future Planning - Palliative Care template in one of the following ways:
- Click on the Run Template option with consultation.
- Click on the Add > Data Using Template option outside of a consultation.
Clicking on either of the options above will display the Template Picker screen. Search for the 'Future Planning - Palliative Care (Ardens)' template using the search box provided or folder navigation pane. Select the template and click on OK.
The template has been broken down by pages on the left-hand side, navigate to the page required.
The Introduction & Patient Consent page, displays information on the patients consent to the national Summary Care Record. Enabling consent for the patient will share all information from the template to the patient's Summary Care Record.
The Treatment Escalation Planning page allows you to record details of the power attorney, CPR status and information on the TEP level.
The Other TEP Considerations page includes respiratory considerations and general information.
The My Wishes page allows you to record the patient's preferred place of care and death, along with other considerations of care after death.
The Frailty page includes options to calculate the patients frailty score and record clinical diagnosis.
The Prognosis & GSF Meeting page includes options to add the patient to the practice GSF/EOL/Frailty registers.
The End of Life/GSF Register page includes fields to complete the end of life care information.
Complete the template using the required fields and ensure you save the information using the Save Template option.
Accessing the Future Care Plan Document
Ensure your consultation is saved first if using the Future Planning Template above.
Access the document template by clicking on Document > Create Letter.
The New Patient Letter screen will display, click on the magnify glass to search template.
This will launch the Find Document Templates screen. Search for the 'Future Care Plan' template using the search provided or folder navigation pane (this can be found in the 'Misc' folder within the Ardens docs folder). Select the template and click on OK.
Please note - the Treatment Escalation Plan includes the latest 5 entries for the patient. If the patient has less than 5 entries, this may duplicate the information. Amend as appropriate.
The document template will merge the relevant patient information. Use the additional tick boxes and text fields to complete further information.
Once complete, click on the File option to Save and Close to the patient record. Additional options such as 'Print' is available.
