Ardens has created a Care Navigation Template to support Reception staff in capturing initial contact information and to aid with the triage process.
Locating the Care Navigation Template
Access the relevant patient record and click on the Add button on the EMIS ribbon, followed by Data Using Template option.
This will open the Template Picker screen. If you have accessed any templates before, these will display to the left of the screen:
Type 'Care Navigation' in the search field and click on the magnify glass icon (alternatively you can click on the Ardens folder on the right hand side > Other Templates > Social folder).
The results will return the 'Care Navigation (Ardens)' template (if you are unable to locate the template, check with your Practice Manager/IT Manager that this is activated in the background).
Select the template and click on OK to open.
Capturing Data Using the Template
When you launch the template, you may be presented with the 'Consultation Properties' screen, here you may wish to change the Consultation Type.
The template has been divided into pages, to make it easy to navigate. Please remember you only need to capture the data needed at that time.
The Template Information page, will not be visited often but does include a link to the Ardens support desk if you have a query.
The Administration Notes page contains data fields to confirm the patients identity, whether they have used an online consultation and if they have a nominated pharmacy. Options for the method of the enquiry e.g. email, telephone, as well as the nature of the call can also be found (you may select more than one option).
To complete data, use the tick boxes and drop down fields provided. The right hand side of each data field will display the latest entry if found in the patient record.
The Nature of problem page is mainly free text (with some drop down options) for you to type in information the patient has given to you. It is always worth informing the patient that you are asking these questions so that you can direct them to the most appropriate clinician to help with their enquiry.
The Outcome page is for you to record your actions. If you feel the call is very urgent and needs to be dealt with quickly, always action this according to your practice protocol. Use the drop down options, tick boxes and free text boxes to record the outcome of your call. The Signposting section allows you to record that you have signposted the patient to an appropriate person/clinician or service, using the drop-down lists.
The Sick Note Request page consists of free text boxes to record the start and end dates as requested by the patient. It include the reason for the patient being off work, as well as how the patient wishes to receive the sick note. Ticking the box below this will prompt the user to open and complete a task to the GP for the sick note to be issued. Alternatively, inform the GP according to your practice protocol.
Complete the template using the required fields and ensure you save the information using the Save Template option.
If using the template via the Run Template option within an open consultation, you will also need to click Save to save the consultation.
