The Ardens Sexual Health template allows the detailed recording of a patient’s sexual health and status, in relation to a wide range of potential diagnoses of sexually transmitted infections.
Access the template in one of the following ways:
- Click on the Run Template option within the consultation.
- Click on the Add > Data using Template option outside of a consultation.
Clicking on either of the options above will display the Template Picker screen.
Search for the 'Sexual Health’ template using the search box provided or via the navigation pane. Select the template and click on OK.
The History page allows the detailed recording of sexual activity, symptoms, contraception and pregnancy.
There are also sections in which to record any safeguarding or domestic abuse concerns.
Please note that Ardens has provided separate templates for both Adult and Child Safeguarding, as well as for Domestic Violence and Abuse.
The Examination page is used to record observations such as pulse, temperature, appearance and urine pregnancy test.
The Investigations page allows the user to record any investigations that have been done, or that are planned or requested.
The Impression/diagnosis page enables the adding of one or more diagnoses codes to the patient record, as well as recording contacts the patient may have had with other people with these conditions.
The HIV page allows for the recording of HIV screening that is accepted or declined, HIV status and monitoring, as well as use of PEP and PrEP medications.
Advice given and other details of a management plan can be recorded on the Management page.
The template includes a page for Fraser Guidelines and Gillick competency.
An onward referral may be recorded on the Referral page.
There are various links to resources for patients on the Resources page, as well as a link to the BASHH patient information leaflets.
Complete the template using the required fields and ensure you save the information, using the Save option.
If using the template via the Run Template option within an open consultation, you will also need to click Save to save the consultation.
