The Ardens Condition searches have been designed to support GP practices with the management of specific diseases, including identifying certain trends and highlighting clinical areas that may need attention. 

 

To view activity for diabetes, navigate to the Population Reporting module > Ardens  Searches > 4.12 Conditions – Diabetes folder.


 
 

 

The Activity Last Month folder provides activity on diagnosis and reviews provided last month. Practices can see this information by Type 1 Diabetes, Type 2 Diabetes, Gestational diabetes, and Prediabetes.

 

 


The Alerts folder identifies potential patient safety issues which clinicians may wish to address. For these particular searches, patients to be looked into will display in the Population Included tab.

 

 


The Ardens Manager service report folder contains a report which can be uploaded to Ardens Manager to view data in dashboard format.  For more information please see the following support article.


The Case Finders folder identities patients who are not correctly coded. Practices are advised to review the list of patients and add the correct disease code to ensure the patient is subsequently reviewed and managed correctly. The Continuous Glucose Monitoring folder identifies diabetic patients who may meet the eligibility criteria for prescribing a continuouse glucose monitor on the NHS.  More information on these searches can be found here.


 

The NDPP/LCD/Digital weight management referrals folder is broken down by subfolders.



The LCD or Pathway to Remission - potentially eligible folder will help practices identify patients with Type 2 Diabetes who may be eligible for referral to the NHS Type 2 Diabetes Path to Remission Programme. These patients’ records must be clinically checked before referring to ensure that referral is appropriate.

 

Type 2 diabetes can usually be prevented, and the National Diabetes Prevention Programme (NDPP) search folder supports practices with monitoring their diabetic and prediabetic patients.   The folder contains Audit, Housekeeping and Invitation searches.  More information on these searches can be found here.


The Weight management ES eligibility folder identifies patients with either Diabetes or Hypertension eligible for referral under the weight management enhanced service. Due to their conditions, they qualify for referral through the NHS Digital Weight Management Programme.

 

The Performance Indicators main folder can be used to track where certain elements of care has not been completed, for example diabetic patients who have not had foot screening or other tests recorded in the last 13m.  Practices can see a breakdown of this information by Type 1 and Type 2 diabetes, gestational diabetes, and prediabetes.  

We have used a 13 month timeframe for these searches as many guidelines and contracts are for activities to be done annually or within a financial year.  If we kept our searches to 12months it does not allow for much leeway for those patients unable to attend exactly on their annual recall date and as these searches are performance indicators and not for contractual payment we felt this timeframe would give a true representation of how the practice is performing. 

  


This folder contains a further 5 sub folders.



The Early Onset Type 2 Diabetes (T2DAY) folder provides a register of patients with Type 2 diabetes for this younger age group, as well as performance indicators and outstanding KCP.

 

The Key care processes folders contain a suite of reports to look at completion of KCP and treatment targets within the  last 13m, 15m and fiscal year.

 

The Treatment Targets folder identifies all patients on the Diabetic register.  The child searches then display patients who have achieved the target for BP, Cholesterol and HbA1c. 

 

The Population Excluded tab will display patients who have not achieved these targets and you may wish to follow these patients up.

  

 

Detailed registers for diabetic patients can be accessed in the Registers folder and go over and beyond the standard QOF registers.

  

 

 

Please note - We advise practices not to use the zSubreports search folder, these are searches which the searches in the main folders are built upon.
