Ardens has created a suite of searches to support practices who are piloting the 18-39 CVD risk project.


To locate the searches, access the Population Reporting module > Ardens Searches > 5.37 Contract – 18-39 CVD Risk Pilot searches folder.


 

1. Invitation Searches

This search folder provides sub folders to help practices with inviting in eligible patients.



The 1a. Send invite folder will identify patients who are eligible for a health check and have not had an invite in the last 5 years.  the folder is further broken down by contact method.


The 1b. Consider invite folder will identify patients on the EOL, LD, SMI or Dementia register. Despite their inclusion on these registers, they remain eligible for a health check.  Some practices may choose not to invite them, therefore separating them from the main invite searches enables this. Other practices may wish to invite these patients,  but may wish to customise invitations or may wish to invite some conditions but not others, the report enables you to export and filter data to accommodate this if needed.



The 1c. Consider visit folder contains very similar searches to the above, however breaks it down by patients in a Care Home, or patients coded as Housebound.



2. Data Quality

This search folder provides searches around coding.  There are searches for ethnicity coding, you may wish to review these patients records and add/amend ethnicity coding.  Additional searches will identify aged 18-39 where an NHS Health Check invitation or done code has been used, you may wish to amend these codes to record a cardiovascular disease risk assessment code.




3. Activity - Quarterly 

This folder includes searches to identify those eligible for cardiovascular disease risk assessment and searches reporting activity done in the last quarter.



4. Audit Report 18-39 CVDRA

This  folder contains a quarterly audit report to allow you to report on all activity.
