Ardens has created a wide range of clinical templates to assist with data entry for a variety of clinical areas.

 

The available Vascular Templates include: 

  • Abdominal Aortic Aneurysm (AAA) - this template enables recording the symptoms of symptomatic or ruptured AAA, the examination of the patient, investigations done, management of the patient and guidance on when to refer. 
  • Giant Cell Arteritis - GCA mainly affects older patients and as many of the symptoms are non-specific this template may work as an aide-memoire to ensure no symptoms are missed. History, examination, investigation and management of the condition can be recorded. There is also a page to record a review with a patient who is a known GCA patient.
  • Varicose Veins - patients of different ages may present with Varicose veins, this template provides a quick entry page to ensure all the required information from a standard consultation can be recorded. There are also more detailed pages including those to record history, examination and recommended investigation. There is a diagram to document the distribution of the varicose veins (very useful if a camera is not available or appropriate) and a page to record the management of the condition.

 

The Vascular templates are located in the Ardens Live Toolset > Consultation Templates folder.

 

They may be accessed within the patient record in one of the following ways:

  • Click on the Run Template option within the consultation.
  • Click on the Add > Data using Template option outside of a consultation.

 

Clicking on either of the options above will display the Template Picker screen. 



Search for the template you require using the search box provided or via the navigation pane. Select the template and click on OK.

 

Alternatively, you can launch the template using the Template Launcher (All Template Launcher or zTemplate launcher: Vascular protocols)

 

Once you have launched the template, complete the template using the required fields and ensure you save the information, using the Save option.

 

If using the template via the Run Template option within an open consultation, you will also need to click Save to save the consultation.
