Ardens has designed New Patient resources to help practice staff capture relevant information for new patients joining the practice. The resources consist of:

  • PRF1 Aligned New Patient template - a clinical template to align with the PRF1 form
  • New Patient Registration Form - a document template for the patient to complete at point of registration (to print blank copies, please see link at the bottom of this article)
  • New Patient template - a clinical template to easily record the information gathered by the New Patient Registration Form (Please note: there are no plans to remove this template)


PRF1 Aligned New Patient (Reception Admin) Template

Ardens has created a new clinical template to align as closely as possible to the paper registration form or PRF1 introduced in October 2024.


The template can be located in Shared Folders > Ardens Live National Content > Reception Admin Templates folder.


You can also launch this and many other Reception / Admin templates from the Reception Template Launcher - an F12 protocol that allows the quick and easy launch of templates for users in reception or admin roles.  You can read more about template launchers here.


The PFR1 template allows practice staff to input, to a new patient record, data such as next of kin, ethnicity, language, carer and pharmacy information, communication consent and patient health details. There is also a page specific to patients from abroad in respect of possible payment for NHS services, S1 forms and EHIC cards.



Please note that for a new patient aged under 18 years, there is an additional page titled Patients Under 18, which facilitates the entry of data such as country of birth, schooling, other professionals involved in care, and routine vaccination status.




New Patient Template

The New Patient Template can be used to capture data such as the patient's family history, lifestyle information and 3rd party consent. Please continue to use the EMIS Registration module to complete the registration, carer and next of kin details. 


Use the Reception Template Launcher protocol, or you can select Add > Data using template from the patient record to access the Template Picker screen.  You can then find the template in Shared Folders > Ardens Live National Content > Reception Admin Templates, or by using the search box.  Select the template and click OK.


The template will open up a new consultation and the Consultation Properties box will appear. Admin staff will usually change the Consultation Type to Administration Note or an alternative appropriate entry in accordance with your practice protocol.  By ticking the Store as default for this session box, you will not need to complete this box again for the rest of  your logged in session.


 

The template has been divided by pages on the left hand side, to facilitate easy navigation. Please note – you do not need to fill in every page.  You only need to complete the data that is necessary at the time.



Each page allows you to record specific information.  For example, the Background page allows you to record information such as Registration details, Gender and Next of Kin.


The Other Details page allows you to record such details as country of birth, ethnic origin, religion and residential status and more if required.


The Communications page allows you to record the patients communication consent, spoken language and any communications needs they may have, and carer details can be recorded on the Carer status page.


The Medical History page allows you to record medical and family history, allergies, vaccinations, EPS consent and cervical screening information.



You can record Disabilities, Lifestyle information and Full AUDIT alcohol disorders identification test on the appropriate pages.   


Please be aware that red text and/or an asterisk denotes contract requirements i.e. entries that are required for payment purposes.

 

The Further Details page can be used to record if the patient is digitally literate, record the Named Accountable GP, if the patient has been asked to take part in the PPG, or if they are willing to be an organ donor.



The Consent page can be used to record the patient’s consent or dissent to share SCR, local shared record and GPDPR, and the 3rd Party, LPA and DNACPR page can be used to record if the patient has allowed access to their records by a 3rd party or if they have a POA.

New Patient Document Template


Ardens has designed a printable registration form to work in line with the New Patient template. It incorporates the areas mentioned above, to make capturing new patient information simpler.  

 

Practices may wish to print a batch of New Patient Registration forms to hand out/send with the GMS forms. To do this, please access the Printable New Patient Registration Form.
