Ardens has created a wide range of consultation templates to assist with data entry for a variety of clinical areas.
The available frailty templates include:
- Falls Assessment - this template covers the assessment and management of older people (>65) living in the community, and covers ‘simple’ falls. There are falls risk and fracture risk assessment tools, patient resources, and a page to record management plan and referrals made.
- Frailty and Over 75s Review - this template allows the user to record comprehensive details of overall health in an over 75 patient, including mobility, falls, frailty, dementia, as well as recording care plan details and medication reviews
The Frailty templates are located in the Ardens Live Toolset > Clinical Templates folder.
They may be accessed within the patient record in one of the following ways:
- Click on the Run Template option within the consultation.
- Click on the Add > Data using Template option outside of a consultation.
Clicking on either of the options above will display the Template Picker screen.
Search for the template you require using search box provided or via the navigation pane. Select the template and click on OK.
Alternatively, you can launch the template using the Template Launcher (All Template Launcher or zTemplate launcher: Frailty protocols).
Once you have launched the template. Complete the template using the required fields and ensure you save the information, using the Save option.
If using the template via the Run Template option within an open consultation, you will also need to click Save to save the consultation.
Additionally, there is a protocol that will alert staff if an eFI score has been recorded without a subsequent frailty code.
This protocol is installed inactive by default.
To activate the protocol access the Resource Publisher module > Shared Folders >Ardens Main > Ardens Live > Optional Protocols > QOF Box Protocols folder.
Locate the eFI score and no frailty coding alert protocol
Right click on the protocol and select the option for Activate. The protocol will now be active for all users.
