Ardens has created a wide range of clinical templates to assist with data entry for a variety of clinical areas.
The available Urology Templates include:
- Balanitis - this template has a quick data entry tool (page) to speed up recording of a consultation for a patient with a suspected balanitis condition. For a more comprehensive consultation there are pages to record full history and examination, including recording if a chaperone has been offered. Any investigations or referrals can be recorded, and details of any safety netting advice given.
- Erectile dysfunction - a template to record a consultation with a patient suffering with erectile dysfunction. It allows for recording of history and examination, provides patient questionnaires, medical management information and advice given or referrals made.
- Haematuria - this template can be used when seeing a patient complaining of visible or non-visible blood in their urine. It can be used to record relevant history and examination of the patient, recording urinalysis results, or that a sample has been sent to the lab. A follow up plan can be recorded, with NICE guidance on referral if you are suspicious of cancer as a cause.
- Lower urinary tract symptoms in men - this template has a quick data entry tool (page) to enable clinicians to speed up data entry during consultations for men presenting with LUT symptoms. For a more comprehensive consultation there are pages to record full history and examination, including recording if a chaperone has been offered and a page to record urinalysis or MSU sent off to the lab. Any management advice and referrals made can be recorded in the template.
- Phimosis and Paraphimosis - to record any problems that might occur from a tight foreskin including blood in urine or balanitis and management of the problem or any referral deemed necessary. There is also a "quick" entry page for the more standard consultations.
- Prostatitis - there is a quick data entry tool (page) in the template for a consultation with a patient presenting with ACUTE prostatitis. For a more comprehensive consultation there are pages to detail the conditions that might suggest prostatitis, examinations and investigations. There is also a lifestyle page e.g. BP, height and weight information. Management and referrals details can also be noted.
- PSA Monitoring - a simple template to record the latest PSA results and the ability to record a future diary entry.
- Scrotal pain and swelling - this template also provides a page for a quick data entry tool for consultations with patients presenting with suspected scrotal pain and swelling. For a more comprehensive consultation there are pages to record assessments including general observations and investigations e.g. urine samples etc. There are pages to record management of the problem and referrals made including if the patient is referred to a Fast Track clinic.
- Urinary incontinence - a template for patients presenting with urinary incontinence, including pages to detail history of the condition and investigations. Examination can be recorded and details of whether a chaperone is required. Management recommendations including advice about caffeine intake and any referrals e.g. bladder and bowel services or Fast Track made can also be recorded.
- Vasectomy - a template that can be used by practices that have a vasectomy service or to record any post op reviews done. There are pages to record assessment, examination and counselling and consent. There is a page to record the procedure, including after care advice and a page to record the post op review including a plan or next steps. Semen analysis after the vasectomy can also be recorded in this template.
All the templates have hyperlinks to useful websites and patient information leaflets.
The Urology templates are located in the Ardens Live Toolset > Clinical Templates folder.
They may be accessed within the patient record in one of the following ways:
- Click on the Run Template option within the consultation.
- Click on the Add > Data using Template option outside of a consultation.
Clicking on either of the options above will display the Template Picker screen.
Search for the template you require using the search box provided or via the navigation pane. Select the template and click on OK.
Alternatively, you can launch the template using the Template Launcher (All Template Launcher or zTemplate launcher: Urology protocols).
Once you have launched the template. Complete the template using the required fields and ensure you save the information, using the Save option.
If using the template via the Run Template option within an open consultation, you will also need to click Save to save the consultation.
