Ardens has created a wide range of clinical templates to assist with data entry for a variety of clinical areas.

 

The available Palliative Care Templates include: 

  • Anticipatory Care - this template provides advice and guidance to assist clinicians in starting safe doses of end of life anticipatory medications. There are links to futureplanning.org.uk and the code to record that a patient has moved to palliative care anticipatory medication.
  • Future Planning (including PCSP) - to document the management of care and preferences for palliative care patients, this template has been developed in association with the Future Care Planning Project. The initial page allows for the recording of consent/decline to upload personal medical information to the SCR. Subsequent pages capture TEP information, patient preferences (such as the preferred place of care and place of death), frailty stage, prognosis, and GSF details. The final page includes the Karnofsky performance status scale.

Upon filing the template, a document can be generated for the PCSP – search for Future Care Plan in the document picker.

 

The Palliative Care templates are located in the Ardens Live Toolset > Clinical Templates folder.

 

They may be accessed within the patient record in one of the following ways:

 

  • Click on the Run Template option within the consultation.
  • Click on the Add > Data using Template option outside of a consultation.

 

Clicking on either of the options above will display the Template Picker screen. 

 


Search for the template you require using the search box provided or via the navigation pane. Select the template and click on OK.


Alternatively, you can launch the template using the Template Launcher (All Template Launcher or zTemplate launcher: Palliative Care protocols). 

 

Once you have launched the template. Complete the template using the required fields and ensure you save the information, using the Save option.

 

If using the template via the Run Template option within an open consultation, you will also need to click Save to save the consultation.
