Ardens has created a wide range of clinical templates to assist with data entry for a variety of clinical areas.

 

The available Pharmacy Templates include: 

  • Adrenaline Auto-injections - to record a patients need for an AAI, the type they need and an easy and structured way to note the information and guidance provided to the patient. There are links to useful resources for the clinician and information leaflets for patients.
  • Anticoagulation (Warfarin DOAC) - the recording of Warfarin and DOAC dosage and need are part of the GP contract and DES. The initial page provides the ability to document all of the information to meet these requirements. Subsequent pages include the assessment for need of anticoagulation medication and the initial issue and monitoring of Warfarin and DOAC, including any need to switch. The Creatinine Clearance calculators are also available, both the link to the online calculator and the integral EMIS calculator.
  • Benzodiazepines + Z-drugs - a template to help manage patients who are prescribed Benzodiazepines and Z-drugs, including assessment and prescribing information of the drugs with links to guidance from NICE and MIND. Subsequent pages include recording lifestyle information and advice and guidance on withdrawal.
  • Corticosteroid -a straightforward single page data entry template to record initiation and monitoring of a corticosteroid, recording examination of the patient and details of their latest blood results. The final section enables recording any management and advice given with links to useful patient information leaflets
  • Covert Medication administration - this template can be used to help consider if a patient is deemed appropriate for Covert Medication.
  • DRUM Review - enables dispensing practices to record a dispensing review of medication including compliance and concordance.
  • Drug Monitoring - smart working template displaying only the pages relevant to the prescribed drugs of the selected patient. Each drug has an individual page to record the drug review, displaying green ticks or red crosses to indicate whether the relevant monitoring has been completed. There are pages to record initial/review appointment details.
  • Medication Safety Review - designed for use in conjunction with Ardens Prescribing Searches. This template may be useful to remove the patient from the denominator searches when a review has been completed to prevent them from appearing in the review list unnecessarily. There is also a page to calculate creatine clearance.
  • Pharmacist Review - Pharmacists may find this template useful to record a review consultation, including the type and reason for consultation, the medication review and outcomes. There is also a more general lifestyle information page.
  • Pharmacy First Service - this service encourages patients to see a Pharmacist for more minor conditions. The template details which conditions/problems may be suitable for a community pharmacist consultation. The template prompts for the details needed for the referral, when the template is saved the end user will be prompted to merge these details into a pre-formatted Word document if they have accepted the referral.
  • Pharmacy Technician Review - similar to the pharmacist review template but including a device review and outcomes section.
  • Structure Medication Review - a structured medication review is needed for certain groups of patients as a requirement for Network DES. This template enables accurate recording of the review. The template includes a page for a medication review, DRUM review, the multi-compartment assessment and STOMP. There is also a page to record general lifestyle information, management and a page providing the latest relevant blood results for the patient.

 

The Pharmacy templates are located in the Ardens Live Toolset > Clinical Templates folder.

 

They may be accessed within the patient record in one of the following ways:

 

  • Click on the Run Template option within the consultation.
  • Click on the Add > Data using Template option outside of a consultation.

 

Clicking on either of the options above will display the Template Picker screen. 

 



Search for the template you require using the search box provided or via the navigation pane. Select the template and click on OK.


Alternatively, you can launch the template using the Template Launcher (All Template Launcher or zTemplate launcher: Pharmacy protocols). 

 

Once you have launched the template. Complete the template using the required fields and ensure you save the information, using the Save option.

 

If using the template via the Run Template option within an open consultation, you will also need to click Save to save the consultation.
