Ardens has created a wide range of clinical templates to assist with data entry for a variety of clinical areas.

 

The available Neurology Templates include: 

  • Blackouts and Syncope – enables you to easily capture patient symptoms, history and assessment for a suspected syncope or blackout.
  • Dementia review - has the codes needed to record the dementia care plan or review needed for GP (QOF) Contract but also makes recording all the other discussions and investigations with a patient with dementia including mental health, frailty and cognitive assessments scores, capacity and future planning easier.
  • Dizziness – includes a quick entry page to record observations and safety netting, along with details about how to manage the different conditions that may involve dizziness. 
  • Epilepsy review - this template has a smart page for the GP contract requirements. A more general consultation can be recorded in the following pages e.g. seizure information, lifestyle information. The resources page has links to useful professional and patient resources and statements that may be useful to copy in a text message to the patient.
  • Head injury and concussion – includes follow-up safety netting with links to support head injury and concussion advice.
  • Headache – one of the most common neurological problems present to GPs and neurologists. This template includes links to the “Headache Red Flags”, along with a useful headache diary to be completed by the patient. 
  • Migraine – enables you to capture information about the suspected or established diagnosis of migraine, with links to management guidelines. 
  • Multiple Sclerosis – useful to establish a diagnosis of MS and record a review. It includes additional links to DVLA guidance on driving and appropriate referrals to be made.
  • Neurological symptoms – many patients with neurological symptoms will be referred to a specialist for diagnosis and treatment, but others can be managed within Primary Care. This template will allow you to record the relevant information in Primary Care and will link you to guidance on whom to refer to and when. In addition, it alerts you to red flags when recording different symptoms. 
  • Parkinson's Disease – enables you to capture symptoms of a new diagnosis for Parkinson’s disease, with management recommendations for future care / end stage plans.
  • Restless Legs syndrome – useful template to record history, assessment and blood test required for the patient, with links to the RLS rating scale and NICE guidance. 
  • Stroke and TIA review - this template has a smart page with all of the GP contract requirements including anti-platelets therapy, LLT requirements and the cholesterol indicators. The general review and lifestyle pages help go record all discussions had easily including links fatigue tools and links to PHQ and GAD calculators. Management pages to record medication and lipid management.
  • Suspected dementia - recording detailed information is very important for dementia patients this comprehensive template includes links to many dementia and memory tools and places to record these scores, capacity and consent and a page detailing all of the patients most recent blood tests. There is also a page to record the patients EOL wishes.
  • Suspected Epilepsy - a template to use if a definite diagnosis of epilepsy has not been made. Pages to record assessment, lifestyle and management including safeguarding. There are links to patient and professional resources and a suggestion of an information text that could be sent to the patient.
  • Suspected stroke and TIA - assessment for a patient who may had a stroke or TIA and guidance for management for the condition. 

 

The Neurology templates are located in the Ardens Live Toolset > Clinical Templates folder.

 

They may be accessed within the patient record in one of the following ways:

  • Click on the Run Template option within the consultation.
  • Click on the Add > Data using Template option outside of a consultation.

 

Clicking on either of the options above will display the Template Picker screen. 



Search for the template you require using the search box provided or via the navigation pane. Select the template and click on OK.

 

Alternatively, you can launch the template using the Template Launcher (All Template Launcher or zTemplate launcher: Neurology protocols)

 

Once you have launched the template, complete the template using the required fields and ensure you save the information, using the Save option.

 

If using the template via the Run Template option within an open consultation, you will also need to click Save to save the consultation.
