To locate the searches, navigate to the Population Reporting module > Ardens Searches > 5.36 Contracts - NHS Health Check searches folder.
1. Invitation Searches
This search folder provides searches to help practices with inviting in eligible patients. For further information, please see the following support article.
2. Data Quality
This search folder provides searches identifying patients where a health check has been started but not completed, patients who have failed to respond to 3 invitations, a health check completed however the patients were not eligible, and searches identifying patient who have had a health check invitation or health check completed however are outside of the eligible age range.
3. Activity - Monthly
The following folder includes searches to identify NHS Health Check activity done in the last month, with a break down by month and cohorts.
4. Activity - Quarterly
There are searches providing the percentages of NHS Health Checks done, invitations sent, with breakdown by cohorts and outcomes.
5. Ardens Manager NHS HC Contract Report
This search folder contains a report which can be uploaded to Ardens Manager (National Contract Subscribers only) to view in dashboard format. Please see further information in the following support article.

We have also made available an NHS Health Check Invite document template, as well as an NHS HC Results document template. These can be found by clicking from within the patients care record Document > Create letter and searching for the relevant document.
