Ardens has created a suite of Learning Disability searches to assist practices with the management of Learning Disability patients.
To locate the searches, navigate to the Population Reporting module > Ardens Searches > 4.15 Conditions - Learning Disability folder.
Catch-up LD health checks
This folder contains searches identifying Learning Disability patients who have not had an annual health check this fiscal year.
Ethnicity Reports for LD Patients
The Ethnicity Reports for LD Patients folder contains searches to identify Learning Disability patients with missing or incorrect ethnicity codes.
Alerts
This folder contains searches to identify Learning Disability patients who have been admitted or discharged from hospital in the last 13m, but no post discharge review has been recorded. Practices may wish to review these patients records and contact them for a post discharge review.
The Alerts folder also includes STOMP searches, displaying patients on certain medications without a medication review in the last 6 months and a potential missing diagnosis code. Practices may wish to contact these patients for a review:
Health Screening
This folder contains searches identifying LD patients potentially eligible for AAA screening or diabetic retinopathy screening.
There is also a search displaying LD patients with diabetes whose latest retinopathy screening code is declined, refused, or not indicated, practices may wish to review these patients records and discuss this with the patient:
Performance
This folder contains a suite of searches indicating where certain elements of the review is incomplete.
We have used a 13 month timeframe for these searches as many guidelines and contracts are for activities to be done annually or within a financial year. If we kept our searches to 12months it does not allow for much leeway for those patients unable to attend exactly on their annual recall date and as these searches are performance indicators and not for contractual payment we felt this timeframe would give a true representation of how the practice is performing.
Registers
This folder provides a breakdown of LD patients with Autism and a register of all your LD patients (including left and died for audit purposes):
To run the searches - right click on the required search and select the option for Run, (alternatively, right click on a search folder and select the Run option, this will run all searches contained in the folder).
Once the searches have been run, patients that meet the criteria will be included in the Population Included tab for each search.
