Ardens has created various frailty resources to support practices with the identification and management of frail patients. 


The searches to support this can be located in the Population Reporting module > Ardens Searches > 4.11 Conditions - Frailty, CH + EoL > Frailty Management (and identification) folder.



Identification

The following search folder will indicate possible frail patients based on the latest eFI or Rockwood score. 



Patients included in the '*Latest eFI....' searches will need to be clinically reviewed before the appropriate frailty code is added to the patient record. The Ardens 'Frailty and Over 75s Review' template can be used to add the codes.



Patients included in the '*Patients eligible for frailty score' search will produce your frailty register. It will show those patients eligible and those that have never had a score recorded. 


Management

The Management folder is broken down by two sub folders (Severe and Moderate) and provides searches to identify where certain activity has not been completed in the last year.



Once the searches have been run, the patients that meet the criteria will be displayed in the Population Included tab.
