It is recommended that all clinical commissioning groups (CCGs) support primary care networks (PCNs) to implement blood pressure (BP) monitoring @home for patients with a diagnosis of Hypertension, to allow treatment to be optimised where necessary. Further NHS England guidance can be found here.
Identification of Target Populations
To support practices with identifying Hypertension patients by 'at-risk' groups, the following Ardens searches are available.
