It is recommended that all clinical commissioning groups (CCGs) support primary care networks (PCNs) to implement blood pressure (BP) monitoring @home for patients with a diagnosis of Hypertension, to allow treatment to be optimised where necessary. Identification of Target Populations

To support practices with identifying Hypertension patients by 'at-risk' groups, the following Ardens searches are available.

To access the searches, navigate to the Population Reporting module > Ardens Searches > 2.1 Ardens Risk Stratification searches > Chronic Disease Risk Stratification - UCLP criteria > Hypertension folder.

Ardens Hypertension Template

The Ardens Hypertension Template includes links to patient education guidance and the recording of the BP results.

To locate the Ardens Hypertension Template, access the relevant patient record. Open a new consultation and select the Run Template option (if not in consultations, click on the Add button on the EMIS ribbon, followed by Data Using Template option).

This will open the Template Picker screen. If you have accessed any templates before, these will display to the left of the screen. Search for the template by typing “hypertension” in the search box.


This will return the Hypertension (Ardens) template (alternatively select the Ardens > Chronic disease templates folder to the right of the screen).

Select the BP @ Home page.

Complete the necessary information and Save Template once complete.
