Ardens has created data gathering forms to aid data collection when visiting patients in a care home or in supported living, or patients with learning disabilities living in a similar setting. This is helpful where a clinician does not have access to EMIS Web. 

The documents have been designed to be printed for completion at the location and then following this visit the information is to be captured using the required Ardens templates. 

The available Data Gathering Forms include:

• Care Home data gathering for clinicians (this form can also be used for an acute visit or ward round – please print pages 6 and 7 only).
• Dementia data gathering for clinicians 
• Learning disability data gathering form for clinicians

To use the forms, access the relevant patient record either:

• Within a consultation - select Document > Create letter
• From the patient record - select Add > Document > Create letter

This will display the New Patient Letter screen.

Click on the magnify glass icon to search for the relevant document template.

This will display the Find Document Templates screen.

Search for the document template using the search box provided (alternatively, for Resource Publisher sites, select Shared Folders > Ardens National Documents folder > Data Gathering Forms. For Template Manager sites, under the Hierarchy tab, select the Ardens Docs folder > Data Gathering Forms).

 Double click on the relevant document to launch.

The following example shows the Care Home data gathering for clinicians form.  

Once open within a patient record, the document will include the current date and patient’s details, ready for printing.

Click on the File option at the top left of the screen and select Print. Then click on the File option again and select Close. 

Following the visit, for the quick and easy inputting of the information onto the patient’s record, please use the Ardens 'Care Home Review' template, which can be located via Run Template option with a consultation (or Ardens Live Toolset > Consultation Templates > Social folder). Alternatively, if using the Learning Disability data gathering form for clinicians, please use the Ardens 'Learning Disability' template,  which can be located via Run Template option with a consultation (or Ardens Live Toolset > Chronic Disease Templates).
