Ardens has created a variety of resources to assist practices with identifying patients with a potential safeguarding concern and supporting their ongoing safeguarding management following the latest NICE guidance.
Safeguarding Templates
The following clinical templates are available:
- Adult Safeguarding
- Child Safeguarding
- Domestic Violence + abuse
- Human Rights Abuses
The Adult Safeguarding template facilitates the recording of a general assessment, a review of capacity, the documentation of safeguarding risk factors and concerns, as well as any referrals that are made.
The Child Safeguarding template allows for the recording of social and risk factors, general information about a child’s health and wellbeing, other agencies involved, and guidance on management and reporting of all concerns. The template can also be used to add or remove child safeguarding / protection codes on the patient record.
The Domestic Violence + Abuse template facilitates the recording of the history and type of domestic abuse, reassurance regarding patient confidentiality, social history, examination, disclosures, immediate danger and management. There are many useful and relevant website links on the Resources page.
The Human Rights Abuses template enables the recording of background information (including reasonable adjustments), assessment, planning and safeguarding of the patient.
All the templates include body maps. The Safeguarding templates also include pages for Exploitation and Prevent, FGM (Female Genital Mutilation) and Honour-based abuse. Please note - There is also a separate Ardens template for FGM if required, located in the Obs+Gynae folder).
All four Safeguarding templates are located in the Ardens Live National Content > Clinical Templates > Safeguarding folder.
Access the template in one of the following ways:
- Click on the Run Template option within the consultation.
- Click on the Add > Data using Template option outside of a consultation.
Clicking on either of the options above will display the Template Picker screen.
Search for a 'Safeguarding' template using the search box provided or via the navigation pane. Select the template and click
OK.

The template has been broken down by pages on the left-hand side. Navigate to the page required.
The Consultation Administration page enables documenting of details such as appointment type, who has attended with the child, concerns around the child's presentation and interaction with parent, as well as a section to record other agencies involved.
The Physical Assessment page allows for the recording of history and physical examination and also includes a Body Map which can be easily annotated.
There is a Social page for documenting details such as who the child lives with, social problems, education and smoking and alcohol use.
The Risk Factors page allows for the coding of concerns around risk factors within the family such as domestic abuse, drug and alcohol misuse, also family history of mental health problems.
There are pages to record information relating to Trafficking, Slavery and Prevent, and details of Female Genital Mutilation or Honour-based Abuse. There are many links to useful resources and helplines.
The Coding (safeguarding inc LAC) page allows for the coding of a child who is subject to safeguarding or Looked After Child proceedings. Child protection codes can be added to categorise the protection needs applicable, or to add the child to the Child Protection register. EMIS Web will add child protection flags to the patient demographics bar for these codes. You can remove the flags using the removal codes on this page.
The Management + Referrals page allows for the recording of the impression, management and any referrals undertaken.
Complete the template using the required fields and ensure you save the information, using the Save option.
If using the template via the Run Template option within an open consultation, you will also need to click Save to save the consultation.
Child Safeguarding letters and reports
Ardens has created document templates for Child Safeguarding letters and reports, such as a Child in Need GP Report, a Looked After Child Report, or a Section 47 Enquiry GP Report, for example. Further information on these resources may be found here.
Safeguarding searches and reports
The Ardens Safeguarding searches will support practices in identifying potential safeguarding patients, produces a register of the practices safeguarding patients, and help to manage those patients. The searches are accessible in Population Reporting; more information about these searches is available here.
