Ardens has created a variety of resources to assist practices with identifying patients with a potential safeguarding concern and supporting their ongoing management following the latest NICE guidance.

 

Recording Information on the Management of Safeguarding Patients 

  • Adult Safeguarding
  • Child Safeguarding
  • Domestic Violence + abuse

 

The Adult Safeguarding Template facilitates the recording of a general assessment, a review of capacity, the documentation of safeguarding risk factors and concerns, as well as any referrals that are made.

 

The Child Safeguarding Template allows for the recording of social and risk factors, general information about a child’s health and wellbeing, other agencies involved, and guidance on management and reporting of all concerns.  The template also allows for the adding or removing of child safeguarding / protection codes to the patient record.


The Domestic Violence template facilitates the recording of the type of domestic abuse, reassuring the patient around confidentiality, social history, examination, and management.

  

All the templates include body maps, the Safeguarding templates also include pages for Modern Slavery, Human Trafficking and Female Genital Mutilation. Please note - Ardens also has a separate template for FGM if required.

 

The Safeguarding Templates are located in the Ardens Live Toolset > Consultation Templates > Safeguarding Folder.

 

Access the template in one of the following ways:

  • Click on the Run Template option within the consultation.
  • Click on the Add > Data using Template option outside of a consultation.

 

Clicking on either of the options above will display the Template Picker screen. 


Search for a 'Safeguarding' template using the search box provided or via the navigation pane. Select the template and click on OK.


 

The example below shows the Child Safeguarding Template.

 

 

The template has been broken down by pages on the left-hand side. Navigate to the page required.

 

The Consultation Administration page enables documenting details such as appointment type, who has attended with the child, concerns around the childs presentation and interaction with parent, as well as a section to record other agencies involved.

 

 

 

The Physical Assessment page allows for the recording of history and physical examination and also includes a Body Map which can be easily annotated.

 

 

There is a Social page for documenting details such as who the child lives with, social problems, education and smoking and alcohol use.


The Risk Factors page allows for the coding of concerns around risk factors within the family such as domestic abuse, drug and alcohol misuse, also family history of mental health problems.


There are pages providing information and for recording details of Female Genital Mutilation and Trafficking, Slavery + Prevent, with many links to useful resources and helplines.


The Coding (safeguarding in LAC) page allows for the coding of a child subject to safeguarding or Look After Child proceedings; to categorise the protection needs applicable, or to add the child to the Child Protection register. EMIS Web will add child protection flags to the patient demographics bar for these codes.  You can remove the flags using the removal codes on this page.

 

 

The Management + Referrals page allows for the recording of the impression, management and any referrals undertaken.

 

Complete the template using the required fields and ensure you save the information, using the Save option.

 

If using the template via the Run Template option within an open consultation, you will also need to click Save to save the consultation.

 

Ardens has created document templates for Child Safeguarding letters and reports, such as a Child in Need GP Report, or a Looked After Child Report, for example.  You can read more about these document templates here.
