The Ardens 6-8 Week Baby Check template enables the clinician to record all information relating to the routine detailed physical examination recommended for a baby between 6 and 8 weeks old.
Access the 6-8 Week Baby Check template in the patient record in one of the following ways:
- Click on the Run Template option within the consultation.
- Click on the Add > Data using Template option outside of a consultation.
Clicking on either of the options above will display the Template Picker screen.
Search for the '6-8 Week Baby Check' template using the search box provided or via the folder navigation pane. Select the template and click on OK.
The template has been broken down by pages on the left-hand side to make it easy to navigate to the information you require.
The Birth Details page allows you to view or record details of the child’s birth, including head circumference, weight, maturity and delivery type.
The Social Circumstances page enables the recording of social information, such as who the child lives with, and whether there is smoking or substance misuse in the household. It also allows for the recording of any domestic abuse, FGM, safeguarding concerns, or any other social dynamics of note.
Please note that Ardens has also created templates for FGM (Female Genital Mutilation), and there are also Child Safeguarding resources which include a clinical template, document templates and searches.
The Examination page allows the clinician to record the information collected in the 6-8 week baby check. This information includes head circumference, weight and height, including centiles. The page also allows for the noting of physical details, such as behaviour, eyes, heart, hips and genitalia.
Many of these examination text fields are pre-populated with normal responses, as highlighted below. If there are concerns to note, please delete the default text and replace as necessary.
The Administration page gives the opportunity to record that the 6-8 week examination has taken place. Details regarding immunisation, consent, and parental responsibility can also be recorded on this page.
There are links to signpost the parent to the ICON Programme, and to advise on caring for the baby at night.
The latest SCID (Severe Combined Immunodeficiency) status can be viewed, as well as the recording of whether or not BCG vaccination is required for the child.
There is a link to the ‘When Should I Worry’ booklet, with a tick box to record that a self-help advice leaflet has been given.
