The Ardens Musculoskeletal template facilitates the recording of all information normally gathered when a patient attends an appointment with a First Contact or MSK Practitioner.

 

This includes history, examination, assessment for red flags, investigations, management and referrals.

 

Please note that Ardens has also created a number of other related clinical templates for the following:

 

Access the Musculoskeletal template in the patient record in one of the following ways:

  • Click on the Run Template option within the consultation.
  • Click on the Add > Data using Template option outside of a consultation. 

 

Clicking on either of the options above will display the Template Picker screen. 

 

Search for the 'Musculoskeletal' template using the search box provided or via the folder navigation pane. Select the template and click on OK.

 

The template has been broken down by pages on the left-hand side to make it easy to navigate to the page you require.

 

The History page facilitates the recording of the appointment type, referral source and detailed history, and also includes links to scores and scales for the assessment of the condition.


 

The Examination page allows for the recording of observations and examination findings, along with a link to a Dermatome map.


 

 The Diagram page allows you to document and annotate body maps as part of your patient’s care.


 

There are separate pages for Neck, Back, Shoulder/Elbow, Wrist/Hand, Hip/Knee and Ankle/Foot. These pages allow you to record whether red flags are present.  They also provide links to relevant information on the Clinical Knowledge Summaries service provided by NICE.  Links to related patient information leaflets may also be accessed.

 

 

The Investigations page enables the recording of any further tests that may be required.


 

 

The Management page allows the practitioner to record impressions and advice given, including PRICE/HARM information. Other interventions may also be recorded such as minor surgery, heat or massage therapy and drug prescription.


The Referral page facilitates the recording of a self referral, an onward referral or urgent hospital admission, or a referral within your organisation.
