The Ardens condition searches have been designed to support GP practices with the management of specific diseases, including identifying certain trends and highlighting clinical areas that may need attending. This particular folder focuses on frailty, care home and EOL patients.


To view the activity, navigate to the Population Reporting module > Ardens Searches > 4.11 Conditions - Frailty, CH + EoL search folder. 



The Activity last month and Activity last year folders provides activity on diagnosis, reviews, assessments, visits and screening done in the last month and year.



The Alerts folder identifies possible review for care home patients based on no MDT visits in the last 3 months and patients to possibly deduct as the patient has a deceased code in their record and still have an active registration. 



The Ardens Manager service report is for practices that wish to upload this condition data to the Service Dashboard for further monitoring, please see further information on how to upload in the following support article.


The Care home data quality folder identifies patients with incorrect care home codes for Network DES.


The Case finders folder identifies patients who are not correctly coded. Practices are advised to review the list of patients and add the correct disease code to ensure the patient is subsequently reviewed and managed correctly. The Daffodil reports (RCGP) folder supports practices with analysing and auditing patient deaths. Please see the following support article for further information. 


The Frailty Management (and identification) folder supports GP practices with the identification and management of frail patients. Please see the following support article for further information. 


The Performance Indicators folder can be used to track certain elements of care which has not been completed.

We have used a 13 month timeframe for of these searches as many guidelines and contracts are for activities to be done annually or within a financial year.  If we kept our searches to 12months it does not allow for much leeway for those patients unable to attend exactly on their annual recall date and as these searches are performance indicators and not for contractual payment we felt this timeframe would give a true representation of how the practice is performing.

The Resuscitation folder contains searches to identify patients with a non-compliant DNACPR code.


The Review folder contains searches for after death analysis purposes and shows patients that have passed away in the last 3 months.



The Work to do folder contains searches for care home patients, reporting on activity not captured.



Please note - We advise practices not to use the zSubreports search folder, these are searches which the searches in the main folders are built upon.
