Ardens has created a wide range of clinical templates to assist with data entry for a variety of clinical areas.
The available Cardiology Templates include:
- Atrial Fibrillation - AF a smart national contract page has all the QOF requirements including anticoagulation information. Pages to record history, examination, investigation and screening, a calculation for creatinine clearance, management and useful resources for patients and professionals.
- Chest pain template - this template allows for the recording of history and examination details, along with noting any investigations required. Immediate and longer-term plans can be documented, as well as lifestyle advice given.
- Coronary Heart Disease (CHD) - the smart national contract page includes all the QOF requirements including cholesterol indicators. There are pages to record the management of stable angina and recent MI, as well as latest investigations, examination and history and information about seasonal vaccinations.
- Heart Failure Review - all of the national contact requirements are included on the smart National Contracts page, there are pages to record a more general review (also including QOF requirements) and to record a patients end stage or complications. This template also have text statements that could be copied and sent to the patient.
- Hyperlipidaemia - this template can help to record the necessary details for starting a patient on statin treatment, following a blood profile. It can also assist in recording follow up reviews and referrals.
- Hypertension - guidance and entries for CVD prevention as well as a smart page for all of the national contract requirements, pages to record assessment, lifestyle and management. There are also links for patients and professionals
- Palpitations - the Palpitations template provides pages for recording assessment, investigations and the management of the condition. It also includes NICE guidance on emergency admission and referrals.
- QRisk Data View - a template which contains all the information that is needed to be recorded or taken from the patient record for QRisk and a link to the online QRisk tool.
- Suspected heart Failure - if a definite diagnosis of heart failure this template is an aide memoire of information needed including a quick entry page but also assessment and management pages and online resources for both patients and professionals
Most of the above templates contain a Quick Entry page, to speed up data entry by condensing key information into one page.
The Cardiology templates are located in the Ardens Live Toolset > Clinical Templates folder.
They may be accessed within the patient record in one of the following ways:
- Click on the Run Template option within the consultation.
- Click on the Add > Data using Template option outside of a consultation.
Clicking on either of the options above will display the Template Picker screen.
Search for the template you require using search box provided or via the navigation pane. Select the template and click on OK.
Alternatively, you can launch the template using the Template Launcher (All Template Launcher or zTemplate launcher: Cardiology protocols).
Once you have launched the template. Complete the template using the required fields and ensure you save the information, using the Save option.
If using the template via the Run Template option within an open consultation, you will also need to click Save to save the consultation.
