Ardens has created a wide range of clinical templates to assist with data entry for a variety of clinical areas.

The available Urology Templates include:

Any investigations or referrals can be recorded, and details of any safety netting advice given.
  • Erectile dysfunction - a template to record a consultation with a patient suffering with erectile dysfunction.   It allows for recording of history and examination, provides patient questionnaires, medical management information and advice given or referrals made.

It can be used to record relevant history and examination of the patient, recording urinalysis results, or that a sample has been sent to the lab. A follow up plan can be recorded, with NICE guidance on referral if you are suspicious of cancer as a cause.

Any management advice and referrals made can be recorded in the template.
