Ardens has designed a wide range of clinical templates to assist with data entry for a variety of clinical areas.


The available Miscellaneous Templates include:

  • Animal Bites - this template can be used to record details of animal bites, specifically cat and dog bites.
  • Bariatric Surgery - a template to record consultations about bariatric surgery (Tier 4 obesity service).  It can also be used to record a post-op review after bariatric surgery.
  • Breast Symptoms - this template allows the recording of a wide range of breast symptoms and prompts to consider risk factors for breast cancer.
  • Breast Surgery Referral - gives information about requirements that need to be fulfilled for either a reduction/Augmentation or implant removal/replacement on the NHS including links to guidance.
  • Chronic fatigue - the Chronic fatigue template looks at the assessment and management of suspected/confirmed myalgic encephalomyelitis and chronic fatigue syndrome.
  • Chronic pain management - the purpose of this template is to provide a patient-centred approach and to produce a care plan suitable for each individual’s pain management.
  • Emergency Scenarios - a "quick" entry page to make recording of an emergency consultation easier including the type of event the patient is presenting with, any procedures, outcomes and follow-ups. The template has useful links to the Resus algorithms and other professional resources.
  • General Consultation - a template to record a general consultation. Please note that Ardens also provides many other condition-specific templates that may suit the purpose better.
  • General Examination - a template that provides the basis upon which to record common examination findings, and also includes the option of recording possible investigations which may be considered in primary care (abdominal exam, breast exam, etc).
  • Hernia - this template allows the clinician to record history and examination findings as well as management, safety netting and any onward referrals.
  • Human bites - the Human bites template can be used to record history and exam details of a bite, as well as a management plan, onward referral or emergency hospital admission.
  • Insect bites - this template provides the means to record details of an insect bite consultation.  Please note that there are separate templates for Cellulitis and Lyme disease (tick bites).
  • Insomnia - a template to record details of patients presenting with insomnia.
  • Labiaplasty - details the difference between the procedures of labiaplasty, vaginoplasty and hymenorrhapy and record the patients history.
  • Long COVID - long COVID is still a problem for many patients and this template can help to accurately record history, investigations and management of this condition.
  • Minor injuries + burns - this template covers joint sprains, foreign bodies, burns/scalds and whiplash injuries.
  • Tiredness/Fatigue  - a template for tired, TATT, exhaustion or lethargy, and includes a section on red flag symptoms.
  • TNBI - the TNBI template is used to record consultations with people who are Trans, Non-binary and Intersex, LGBT, have Gender Dysphoria or who identify differently.

The Miscellaneous templates are located in the Ardens Live Toolset > Clinical Templates folder.


They may be accessed within the patient record in one of the following ways:

  • Click on the Run Template option within the consultation.
  • Click on the Add > Data using Template option outside of a consultation.


Clicking on either of the options above will display the Template Picker screen. 


Search for the templat e you require using the search box provided or via the navigation pane. Select the template and click on OK.


Alternatively, you can launch the template using the Template Launcher (All Template Launcher or zTemplate launcher: Miscellaneous ) protocols). 


Once you have launched the template. Complete the template using the required fields and ensure you save the information, using the Save option.


If using the template via the Run Template option within an open consultation, you will also need to click Save to save the consultation.
