Introduction

Practices can record smoking cessation data using various Ardens clinical templates. Additionally, the Yorkshire Smokefree Service Referral form can be completed directly within the patient's records. Please note that consultation data is not needed for this referral service; therefore, you may launch the referral document without completing a consultation.

 

Locating the Clinical Template

The easiest way to locate an Ardens clinical template is via the All Template Launcher Protocol from the F12 key on the keyboard. Double-click the protocol and select the relevant option from the multiple-choice question.


All Ardens Chronic Disease review and Multimorbidity templates contain a section for recording smoking data, including smoking cessation advice.

Alternatively, you may wish to use the dedicated Smoking Cessation template, which allows for more detailed recording and is located within the Clinical Templates > Social & Lifestyle option.

EMIS web does not have the functionality to launch a document from within a clinical template. Therefore, the Yorkshire Smokefree Service Referral form will need to be launched once the consultation has been saved.

 

Locating the Yorkshire Smokefree Service Referral form

The referral form is accessed using your local document launcher protocol on the F12 key. Further information on how to add and use this protocol can be found here.


Select the F12 key on the keyboard.


 

Double-click on the protocol to open a multiple-choice question listing the categories of the referrals.

The Yorkshire Smokefree Service Referral form is located within the Social option.

Single-click the Social option and then the Ref – YSS – Smokefree In Barnsley Referral option.

The referral form will then open with the patient's demographic details prepopulated. Additional information can then be completed via the tick boxes and free text field. The name and job title of the refer have not been prepopulated as this may differ from the person completing the form.

To save the referral form to the patient's record, select Save and Close at the top left of the EMIS screen. This will automatically add the code ‘Referral to stop-smoking clinic’ to the patients record.

This referral form needs to be emailed to YSFBarnsley@swyt.nhs.uk. If your NHS email account is linked to Outlook, you can email directly from EMIS.

 

Double-click the referral form from the patient's record – either from the consultation or the documents tab. This will open the referral form, and the option to Send is at the bottom of the page. Click the drop-down arrow and select via Email.

This will automatically open an email in Outlook, attaching the referral form for you to send to the referral service.

If your NHS mail is not linked to Outlook, you would need to use the Export option to export the form and attach to an email in the usual way.
