Ardens has created a wide range of clinical templates to assist with data entry for a variety of clinical areas.


The available Dermatology Templates include:

  • Acne - this template gives background information of the condition and allows the user to record full consultation details, including management, follow up and safety netting, as well as any referrals made.
  • Eczema and dermatitis template - this template defines the condition and focuses primarily on atopic eczema, although there are links to factsheets for more information on other types of eczema. Assessment tools and a body map are included.   Management of the condition, counselling provided and referrals made can all be noted.
  • Fungal skin and nail infections template - this template can be used to record details of suspected fungal skin or nail infections, and to record when skin, nail or hair samples have been taken.  It can also be used to record details of other conditions such as suspected scabies.
  • Psoriasis template - the Psoriasis template allows the clinician to record information of the patient's general history, examination findings and management advice.
  • Skin Lesion/Rashes template - this template can be used to aid assessment and documentation of skin rashes and skin lesions.  Information regarding the recognition of suspected skin cancers is also included.
  •  Wound Management and Leg Ulcers - the Wound Management and Leg Ulcers Template allows the user to record comprehensive details of wounds and leg and pressure ulcers, including wound infection, Tetanus status, referrals and management plans.

 

 The Dermatology templates are located in the Ardens Live Toolset > Clinical Templates folder.

 

They may be accessed within the patient record in one of the following ways:

  • Click on the Run Template option within the consultation.
  • Click on the Add > Data using Template option outside of a consultation.

 

Clicking on either of the options above will display the Template Picker screen. 



Search for the template you require using search box provided or via the navigation pane. Select the template and click on OK.


Alternatively, you can launch the template using the Template Launcher (All Template Launcher or zTemplate launcher: Dermatology protocols). 

 

Once you have launched the template. Complete the template using the required fields and ensure you save the information, using the Save option.

 

If using the template via the Run Template option within an open consultation, you will also need to click Save to save the consultation.
