Ardens has created a variety of resources to assist with identifying patients living in a Care Home and supporting their ongoing management.

 

Recording Information on the Management of Patients living in a Care Home

The Ardens Care Home Review template has been created to simplify data collection for patients in residential or nursing homes/supported living.  This template incorporates the requirements for the Network Contract DES.

 

Access the Care Home template in the patient care record using one of the following ways:

  • Click on the Run Template option within the consultation. 
  • Click on the Add > Data using Template option outside of a consultation. 

 

Clicking on either of the options above will display the Template Picker screen. Search for the 'Care Home Review (Ardens)' template using the search box provided or folder navigation pane (Ardens > Consultation Templates > Social folder). Select the template and click on OK.

You may wish to use the Universal Care Plan template to record a care plan for your Care Home patients.  This template incorporates all the national schemes requiring or recommending care planning in one place.


Ardens Alerts

The Ardens resources includes Clinical Safety Alerts to flag important MHRA drug safety and NICE guidance information that may be missed and Data Quality Alerts to detect potential missing or incorrect coding. The alerts are triggered on certain activity in the patient record e.g. entering a particular code or prescribing specific drugs. 

When entering a care plan code, if this is an incorrect code for the NCD requirements, a multiple-choice question will be displayed for you to select whether or not you would like the correct code added to the patients record.





Ardens Searches

There are various searches that might be useful to support practices with the care of and contract requirements of patients living in a care home:

  • Ardens LTC Recall System - a month of birth recall system, designed to recall a patient for multiple QOF conditions at once.  Patients living in a Care Home are separated from the main recall searches to assist with how these patients are reviewed.
  • Conditions - Frailty, Care Homes and EOL – searches to identify activity, trends and highlighting clinical areas that may need attention.


Ardens Document Templates

Ardens includes many auto-populating standardised document templates for practices. The Ardens document templates folder includes a Data Gathering Form to aid data collection when visiting patients in a care home.


If you have used the Universal Care Plan clinical template and wish to print out the information, the Universal Care Plan  document extracts relevant information, allowing a care plan to be saved/printed and/or shared.  

 

To access a document template, click Document > Create Letter

 

The New Patient Letter screen will display, click on the magnify glass to search for the required template. 

 

This will launch the Find Document Templates screen.  Search for the relevant document template using the search box provided or folder navigation.  Select the template and click OK
