Ardens has created a variety of resources to assist with identifying patients who may have hypertension, and to support their ongoing management once diagnosed. 

Recording Information on the Management of Patients with Hypertension

The Ardens Hypertension clinical template has been designed to standardise consultations, ensuring clinicians are following the latest national guidelines and recording accurate coding of data. 


To locate the Ardens Hypertension Template, access the relevant patient record. Open a new consultation and select the Run Template option (if not in consultations, click on the Add button on the EMIS ribbon, followed by Data Using Template option).


This will open the Template Picker screen. If you have accessed any templates before, these will display to the left of the screen. Search for the template by typing “hypertension” in the search box.




This will return the Hypertension (Ardens) template (alternatively select the Ardens > Chronic disease templates folder to the right of the screen).

All the Ardens Chronic Disease Templates follow the latest NICE guidance and best practice information. They include a QOF page for capturing QOF requirements and Review pages to complete a structured review.


Any information displayed in red relates to payment purposes such as QOF, PCN DES or local payment. Please note - if the information is relevant for the current financial year, you do not need to duplicate this information. 


The New Diagnosis page provides information relating to diagnosis as well as sections for recording details around history, examination, and investigations. Please note the appropriate diagnosis codes for heart failure can be found within the QOF page.



There are pages to enable you to record information relating to Lifestyle, Alcohol Screening and GPPAQ.

The CVD Primary Prevention page provides information relating to QRISK and links to the online QRISK2 or QRISK3 calculator (alternatively you may wish to access the EMIS QRISK2 template via the EMIS library). The latest QRISK results are also displayed (if available).

NICE guideline NG136 recommends an annual review of care for your hypertensive patients and the Review Control page provides information on what should be included, with sections to enable you to record the information:



The Review & Recall page enables users to record that the annual or interim review has been completed, as well recording a future diary date to arrange an interim review – this will link to our Diary Recall System.  For more information on the Diary Recall System, please see our dedicated Support Article.

As of January 2021, it is recommended that all clinical commissioning groups (CCGs) support primary care networks (PCNs) to implement blood pressure (BP) monitoring @home (BPM@h) for patients with a diagnosis of hypertension, to allow treatment to be optimised where necessary.  The BP@home page provides information around this, links to information for patients and a section for recording average BP results.

Please note - for patients presenting with multiple long term conditions, you may wish to use the Ardens Multi-Morbidity Templates.


Equipment Loan Resources 

Ardens has provided a template to easily manage the loan and return of the equipment within the practice, you may loan a BP machine to your hypertensive patients for them to monitor their BP at home.  

Search for the Ardens 'Equipment Loan' template via the Run Template option (if not in consultations, click on the Add button on the EMIS ribbon, followed by Data Using Template option). Once located, double click on the template to launch. 

Selecting the Home BP Monitor page on the left will open the page to enable you to record that you have loaned the machine, the patient has returned the machine, they have been put on a waiting list, or a section to record results:

The Ardens resources includes the following alert to alert practice staff if the patients latest home BP is out of target


Hovering over the alert will display the date and value.


Ardens Hypertension Searches 

There are various searches that might be useful to support practices with the care of Hypertensive patients.  The searches can be accessed via the Population Reporting module > Ardens Searches folder.

1.1 Chronic Disease (QOF) Recall Syst

This is a month of birth recall system, designed to recall a patient for multiple conditions at once.  These searches will ensure your Hypertensive patients are attending their annual review.



2.4 CHD/HYP/STIA with BP taken this QOF Year

This folder contains searches displaying patients on the Hypertension register, who have not had an annual review this financial year, however they have a recorded BP.  Practices may wish to review these patients to check whether they still need to come in for a Hypertension review.



2.5. Patients with latest BP in QOF yr> threshold

This folder identifies patients who have a condition for which their BP needs to be within a certain range for QOF, however their latest BP this financial year is out this range.

1.2 Ardens Diary Recall Searches

The Ardens Diary Recall System allows practice staff to easily manage diary recalls for ad-hoc tests, including blood tests and interim reviews.  These can be used to ensure your Hypertensive patients are called back for any interim review that is set for them, or any blood tests required:

Ardens has created a suite of searches to assist practices with the management and monitoring of equipment loaned to your patients.  These searches look for patients that have loaned and returned equipment, patients who have loaned equipment and not returned it and patients currently waiting for equipment.

1.6 Ardens Miscellaneous Review Searches

As it is recommended that patients with a diagnosis of hypertension monitor their BP at home, the following reports have been provided to monitor patients BP recorded in preferred time period.


2.1 Ardens Risk Stratification Searches 

The Ardens Risk Stratification searches will display patients on the hypertension register, breaking them down by those at high, medium or low risk to support prioritising reviews or home BP monitoring

5.0 Ardens Case Finders

Ardens have provided a suite of searches to support practices to improve data quality and ensure QOF payments are correctly calculated for your Hypertensive patients.  These can be accessed via the Population Reporting module > Ardens Searches > 5.0 Ardens Case Finders > Hypertension folder:



The searches are reporting on patients that fulfil certain criteria which indicates the patient potentially could be included on the Hypertension register but are not recorded with the correct diagnostic code. For more information, please see our dedicated Support Article.


5.1 Ardens Condition Reports

Ardens have provided a suite of searches to help practices monitor activity, alerts, case finders, performance indicators and register information for various conditions.   Within the Conditions – Cardiovascular folder you will find searches relating to your hypertensive patients:



The Activity Last Month folder provides information around diagnosis, reviews and screening completed in the last month:


The Case Finders folder includes additional searches to increase prevalence and will display patients who do not have a hypertension diagnosis code on their records, however they fulfil certain criteria which indicates potentially they could have hypertension:


The Performance Indicators folder includes searches identifying where certain elements of the hypertension review is missing:


The Registers folder includes additional Hypertension registers which practices may find useful over and beyond the standard QOF register:

Home Blood Pressure Diary Document

Ardens has provided a Home Blood Pressure Diary document template to support patients capturing BP readings at home. To access the Document Templates:

  • Within a consultation, select the Documents > Create letter option.
  • Outside of a consultation, select the Add > Document > Create letter option.


This will display the New Patient letter screen.


Click on the magnify glass to search for the document template:



This will display the Find Document Templates screen.


Search for the Home Blood Pressure document template using the search box provided, select the relevant document template and click on OK to launch (alternatively double click on the document to open):



This can be printed for the patient for them to complete at home.
