Introduction

Ardens provides a range of QOF resources to help GP practices improve prevalence, ensure timely completion of indicators, monitor achievement and streamline patient recalls for annual reviews. The following support article provides information on resources available.


Templates

The Ardens 'Chronic Disease' templates assists GP practices in accurately recording QOF activity and adhering to the latest best practice guidance. 


Each QOF condition has a dedicated template. These templates can be accessed via the 'All Template Launcher' protocol via the F12 key or by searching for the condition when using the 'Run Template' option in a consultation. 



Each chronic disease template includes a 'National Contracts' page for recording all QOF and national contract requirements for the current financial year, as well as 'Review' pages for a comprehensive structured review that incorporates NICE best practice data. 



For patients with co-morbidities, Ardens provides 'Multi-Morbidity' templates designed to support the management of multiple conditions. They consist of two templates, one for HCAs to capture initial examination findings, and another for GPs or nurses to record additional review elements. The templates are smart working and will display relevant pages based on the patient's coded conditions. 



Alerts

The QOF Analyser helps practice staff identify any outstanding QOF indicators for individual patients. It streamlines the number of QOF alerts by consolidating multiple QOF indicators into a single, clear alert, summarising the actions needed for completion.



In addition to the QOF Analyser, Ardens is constantly monitoring clinical data as it is entered into EMIS Web, alerting users to incorrect coding or missing QOF requirements in real-time. This ensures accurate QOF coding as you work, helping to increase prevalence and maximise income. 



Searches and Reports

Ardens searches assist practices in several ways, from identifying patients missing from the disease registers to tracking time-sensitive QOF indicators and facilitating patient recalls for annual reviews. Key searches to support QOF include:

  • Registration Anomalies – identifies patients not fully registered, who are not included in almost all payment searches extractions are based on.
  • Ardens Case Finders  –  identifies patients who do not have a QOF diagnosis code, but have information in their record which suggests they should.
  • QOF Monitor  - designed to align with the EMIS Web 'GP Contract QOF ' searches and can be run when needed.
  • QOF Misc  - identifies missing coding requirements and helps manage specific QOF indicators.
  • Time Sensitive   - monitors QOF indicators with time-sensitive achievement requirements.  
  • QOF Vaccination and Immunisation - identifies missing immunisation coding and helps manage certain QOF indicators.  
  • Chronic Disease Risk Stratification - focuses on chronic disease reviews for higher-risk patients.
  • LTC Recall System - recalls patients based on their birth month for multiple conditions. 


Dashboard

Ardens Manager is a cloud-based analytics tool that helps GP practices, PCNs and ICBs  to analyse primary care needs and activities. The QOF Dashboard provides useful insights into QOF performance, including points and threshold achievements. To sign up for a free trial, click here.
